Insurance information

Do you have Vision Insurance?

Many vision insurance companies are able to control where your eye doctor or optician will have your lenses made.  They simply send them to their in house, wholly owned large laboratories or in network labs.

We are not a network lab for any vision insurance provider, we are an independent laboratory.

This does not mean you can't use your vision insurance benefits to purchase prescription lenses or complete eyewear with us at (sorry, replacement non-Rx sun lenses and complete sunglasses do not qualify for reimbursement).  You can use your benefits in 3 easy steps:

1.   Fill out a claim form. (most of these can now be done online).

2.   Attach your receipt to the claim form (we will provide you with an itemized receipt at the time of purchase as well as with your completed order.  Please note that eyewear packages are not itemized as they are sold as a frame and lens package.  We are happy to work with your insurance provider to answer any questions and ensure that your payment is processed promptly).

3.   Submit Claim Form and Receipt!

It’s that easy!

Below is a list of vision insurance providers with contact information.  Simply click the name below and you will be directed to their claim form page (please note, you may have to log in to your insurance account to submit these forms online.  If this is the case, a link to the providers page is attached):
